Admission will be open to any qualified client (15 years or older) regardless of race, gender, sexual identity, or religion. Application must be completed by, parent, guardian or support care provider. The application will be reviewed, and a decision is made either to invite the client, the client’s guardian(s), and Support Coordinator for an interview or you will be notified that our program is not a good fit to address the client’s needs. Such an important decision is based upon many factors, the information from the application describing the client’s functional and psycho-social abilities/impairments, we look for a reasonable “match” between the particular programmatic strengths of our Day Program and the client’s own needs.
Forms of Payment
Once accepted, we work closely with the family and Support Coordinator to discuss services and individualized goals outlined in the Individualized Service Plan. Be a minimum of 15 years old
Have a primary diagnosis of intellectual or developmental disability or cognitive delay
Be free of aggressive or threatening behavior
Have the ability to ambulate and transfer independently
Have an interest in participating and being an active community member
